I/We hereby declare that the details furnished above are true and correct to the best of my/our knowledge and belief, and I/ we undertake the responsibility to inform you of any changes therein, immediately.
I/ We hereby agree to indemnify and keep indemnified the The Nursery and BabyEats (LIFE STAGES RESTAURANT & CATERING) and each of its staff, supervisors, teachers, assistants or directors/ shareholders, or volunteers or helpers or owners, absolutely harmless fully and effectively indemnified against all actions, claims, liabilities, damages, expenses, costs, charges, fees (including medical, judicial, and attorney’s fees) whatsoever, which are in respect of any amounts which The Nursery and/ or BabyEats (LIFE STAGES RESTAURANT & CATERING) may pay, in respect of medical or other expenses arising from accidental bodily injury to our child.
Parents / Carer indemnify The Nursery and BabyEats (LIFE STAGES RESTAURANT & CATERING) if their child develops a food allergy or intolerance to any of the ingredients used in any of the meals provided at The catered by BabyEats (LIFE STAGES RESTAURANT & CATERING LLC), that is not identified or specified by the parent in the application / sign up form. (Parents are required to declare mild, moderate and severe allergies during the signup and registration to the Nursery Catering Service, as the service does not cater to children with any kind of allergy or intolerance).
I/ We, lawful parent or guardian of the child hereby accept and agree that in case of an accident or injury occurring to or virus/diseases being contracted by the child, the Director, Nursery Manager, Assistant Manager, Nurse, or any persons in charge at The Nursery, shall have full authority to take the necessary decisions to ensure appropriate emergency medical treatment of my child by the Nursery Nurse or if necessary at a government hospital/clinic, if I cannot be reached at the emergency numbers.
